This coming school year, 2013-2014, students attending Sachem High School North and Sachem High
School East (grades 9 – 12) may participate in the extracurricular sports and activities, such as cheerleading and
Arrowettes.
In order to participate in extracurricular sports, cheerleading or Arrowettes, the following steps must be completed:
- A physical examination card must be completed. Physicals are valid for one calendar year. If your child has a physical on record with the school nurse that was completed within the last calendar year, then a green card must be completed. Both the physical exam cards and green cards must be signed by a parent/guardian.
- Physical exam cards and green cards are available in the nurse’s office or can be printed from our website (www.sachem.edu). Please note the physical exam card is two-sided.
- No other forms or doctors notes will be accepted as proof of a sport physical.
- If your child requires medication (epi pen, inhaler, diabetic, etc…), a physician’s order must be turned in with physical card or green card to be cleared to participate.
- Sports physicals will be given at your child’s high school (at no cost) on:
